CONSENT TO THE PROCESSING OF THE PROVIDED INFORMATION

I, acting freely of my own free will and in my own interest, in accordance with Article 9 of Federal Law No. 152-FZ dated 27.07.2006 "On Personal Data", hereby give my consent to the Operator: LIMITED LIABILITY COMPANY "BP-Plast" (LLC "BP-Plast") TIN 1657123963, (hereinafter referred to as the Company), located at: 420095, Republic of Tatarstan (Tatarstan), CITY OF KAZAN, KAZAN, TER. HIMGRAD, 126, ZD. 266D,K, ROOM. 346 for the processing of my following personal data within the following time limits and for the purposes set out below:

Goal 1: Request processing. The list of personal data for which this consent has been given: last name, first name, patronymic, e-mail address, phone number, information specified in the request. The list of actions with personal data for which consent is given: collection, recording, systematization, accumulation, storage, clarification (updating, modification), use, extraction, transfer (provision, access), depersonalization, blocking, deletion, destruction of personal data. Terms of personal data processing: until the purpose of personal data processing is achieved or consent to personal data processing is revoked, or a request is received to terminate personal data processing, or the need to achieve the specified goal is lost.

Goal 2: informing website users about the Operator's news. The list of personal data for which this consent has been given: last name, first name, patronymic, e-mail address, phone number, information specified in the request. Goal 3: analysis of the Operator's Website usage. The list of personal data for which this consent has been given: cookies. The list of actions with personal data for which consent is given: collection, recording, systematization, accumulation, storage, clarification (updating, modification), use, extraction, transfer (provision, access), depersonalization, blocking, deletion, destruction of personal data. Terms of personal data processing: until the purpose of personal data processing is achieved or consent to personal data processing is revoked, or a request is received to terminate personal data processing, or the need to achieve the specified goal is lost. I have read the Personal Data Processing Policy. I am aware that my personal data will be subject to automated processing, that is, processing using automation tools. I have read and agree that this consent is valid until the purpose of personal data processing is achieved or until it is no longer necessary to achieve this goal, or until I withdraw my consent to the processing of personal data, or until I send a request to terminate the processing of personal data. My personal data is destroyed by the Operator in accordance with Part 7 of Article 5 and parts 4, 5 of Article 21 of Federal Law No. 152-FZ dated 27.07.2006 "On Personal Data" within 30 (Thirty) calendar days from the date of achieving the goal or receiving withdrawal of consent to the processing of personal data. Upon receipt of a written request to terminate the processing of personal data, the Operator shall terminate the processing of personal data in accordance with Part 5.1 of Article 21 of Federal Law No. 152-FZ dated 27.07.2006 "On Personal Data" within 10 (Ten) business days from the date of receipt of such request. The specified period may be extended, but not more than 5 (Five) business days if the Operator sends a reasoned notification to the personal data subject indicating the reasons for extending the deadline for providing the requested information. I am notified that after receiving my withdrawal of consent, the request for termination of processing, the Operator has the right to carry out further processing of personal data in accordance with paragraphs 2-9.1 of Part 1 of Article 6 of Federal Law No. 152-FZ of 27.07.2006 "On Personal Data", if such processing is necessary to achieve the goals provided by law, to carry out and fulfill the functions, powers and duties assigned to the Operator by the legislation of the Russian Federation, as well as if there are other legal grounds. I am informed that this consent may be revoked at any time upon my written request sent to the Operator's address: 420095, RT, Kazan, 97 a/z, Standard Prof Plant, or by sending a letter to the Operator's email address: zakaz@standart-prof.ru . I am informed that I have the right to request that the processing of personal data be stopped at any time upon my written request, sent in accordance with the procedure described above. I confirm that when giving this consent, I act freely, of my own free will and in my own interests. I acknowledge that this consent is provided by me to the Operator consciously, specifically, subjectively and unambiguously, I am sufficiently informed about the conditions and purposes of personal data processing. I understand my rights in the field of personal data protection and responsibility for providing false information about myself.
